The goal of the community integrated health program is to prevent unnecessary hospitalization and readmittance, and to connect patients to additional resources in their community. We currently have nurses and community paramedic technicians ready to serve your patients! You will receive a faxed report after each visit so that you may better follow your patient's progress.
